A Self-Help Worksheet For Lower Anxiety and Fewer Chart Fines

(By someone who has been there, done that. For real.)

No doubt that this will not work for everyone – stages of readiness for change and all that – but there were definitely a few things that I learned on my journey that might be helpful for others. I was never really a self-help person or someone who swore by certain methods, but there were definitely tools that I ended up engaging that notably lowered by anxiety (seriously, like I could feel my shoulders relax and my blood pressure drop) and changed the patterns that I was in with regards to charts.

My only goal with this worksheet is to help people get away from a toxic relationship with closing charts.

With the goal of making this tool as helpful as possible, I’m also asking that you help me improve this tool with your feedback and see if it is effective as a quality improvement intervention. If you are a person struggling with charts or the supervisor of a person struggling with charts, please take this less than 5-minute survey before you use the tool; there will be another short survey at the end to measure your experience and get your feedback.

Who should use this toolkit?

This toolkit is intended for use by healthcare providers that have struggled with chart completion or supervisors of healthcare providers that have struggled with chart completion.

What does the toolkit contain?

The toolkit contains a worksheet based on motivational interviewing techniques that individuals or supervisors can use to identify small, achievable goals for change related to chart completion.

How should these tools be used?

The materials in this toolkit can be used to:

  1. Help individuals identify small, achievable goals to improve chart completion
  2. Provide a compassionate, non-punitive framework for supervisors to help supervisees set achievable goals related to chart completion

A copy of the survey instruments and information on adapting them is available in this PDF file. Additionally, to allow you to easily adapt the instruments for use in your organization, they are provided as separate file in Microsoft Word/Excel that you can download on the HIPxChange website.

Development of this toolkit

The Positive Pathways to Chart Completion Worksheet was developed by a clinician (Brittany Allen, MD) at the University of Wisconsin-Madison School of Medicine & Public Health – Department of Pediatrics.

About the Author

Brittany Allen, MD, is an associate professor (CHS) in the Division of General Pediatrics and Adolescent Medicine and a pediatrician at UW Health. Dr. Allen cares for children and adolescents from birth through their early 20s. She treats a wide range of conditions and provides preventive care. Dr. Allen’s special interests include adolescent health and affirming care for LGBTQ+ youth and families.
